FACILITY USAGE REQUEST FORM

The Crossroads Institute has a number of facilities that may be of use to your club or
organization. While fees are minimal, reservations and special arrangements should
be made well in advance of each event.The Facility Usage Request Form should be completed,
signed and then returned with appropriate payment to Sarah Deaton at the Crossroads Institute,
1117 East Stuart Drive, Galax, Virginia, 24333.
